1. Clinically evaluate and stabilise in-coming patients prior or during admission
2. In in-coming and admitted patients, recognize and decide admission or further referral of patients depending on
clinical condition and prognosis
3. Conduct periodic clinical rounds and advise treatment / interventions according to MSF protocols
4. Manage advanced HIV and opportunistic infections with a particular focus on disseminated tuberculosis and
other common OIs, support decisions on ART switches based on Indian/WHO Guidelines.
5. Manage severe acute malnutrition, comorbidities and common adverse events, including drug induced liver
injury, seizures, and others.
6. Provide and supervise other clinical interventions (Vital signs, IV lines, catheters, fluids, biological sampling etc.)
within the scope of the inpatient unit and MSF protocols
7. Advise and contribute to standard operating procedures related to clinical care
8. Resuscitate and / or guide resuscitation of patients if indicated
9. Advise project medical management on medical supply needs for facility
10. Capacity building for the ward team, including MSF Medical Doctors and Nurses, in the management of
inpatients with advanced HIV
11. Support and training including bedside training the management of advanced HIV inpatient management:
12. High Dependency/ICU level care not including use of vasopressors and non-invasive ventilation, intubation or
renal replacement therapy
13. Support management of paediatric advanced HIV admissions of all ages including with severe acute
malnutrition.
14. Support the ongoing development of the unit as a hub for good antimicrobial practice, management of
antimicrobial resistance, palliative care and effective infection control
15. Continue to advocate for a reduction in HIV-associated stigma among local staff and including supporting field
coordinator interactions with local external actors
16. Ensure all approved unit-specific or MSF medical protocols are followed and implemented; develop a new SOP
or protocol for any medical condition as and when needed
17. Ensuring universal precautions are followed at all times and reducing bio-hazard risks and improving infection
control
18. Ensure professional confidentiality is respected
19. Work closely with other members of medical team staff (Nurses for triage and patient flow, Counselors, Health
Promoter, Lab Technicians and Medical Data Processing Officer) to ensure quality medical care.
20. Refer patients to appropriate facilities (imagery, TBDC, tertiary Hospital in Patna) for further investigations and
treatment or special care based on indications set by DWBI Protocols; and follow up the evolution of referred
cases by networking/collaborating with partner doctors.
21. Ensure proper and careful use and check quality of biomedical equipment following the user manual and
protocols and alert supervisor in case of malfunctioning.
22. Participate in clinical as well as medical team meetings and tele medicine meetings
Other Responsibilities and tasks
• Support team members to recognize patients requiring palliative care and identifying patients who are dying
and require end of life care
• Provide appropriate palliative care for pain and symptom management in all patients and those on end of life
care
• Work together with focal point to coordinate multidisciplinary team working (including MHPSS), palliative care
data collection and reporting
• Organize training for the ARTCs staff to build their capacity in order to improve the quality of care
• Assist medical management team and to identify ART failure cases and switching to 2nd or 3rd line or Enhanced
Adherence Counselling and FU VL,
• Maintains a flexible approach and responds to the needs of the project even outside of this job description.
• To assist in the assessment of humanitarian needs and emergency interventions arising in the region and if
needed, showing flexibility, adaptability and commitment.
• To respect MSF internal regulations, the contract and the present job description.
REQUIREMENTS
Education • Essential: DNB (Diplomat of National Board of Examinations) in Medicine / internal
medicine / infectious diseases with full & valid registration from Medical council of
India or state medical councils
• Desirable: MD (Doctor if Medicine) in Medicine / internal medicine / infectious
diseases, with full & valid registration from Medical council of India or state medical
councils.
Experience • Essential: Minimum 4 years of work experience including specialization residency
(DNB or MD) of which 2 years in a in-patient facility treating advanced HIV, TB
and/or MDRTB;
• Desirable: Experience in treatment of visceral Leishmaniasis and conduct of tissue
aspirates techniques for diagnosis
Languages • Advanced skills in English. Spoken Hindi is desirable
